UAID Health Equity Scholarship Agreement (Advanced / Leaders Tier)

1. Parties

This Agreement is entered into between United Against Inequities in Disease (UAID) (“UAID”) and [Name entered into submission form] (“Recipient”) effective October 13, 2025.

2. Purpose

UAID awards the Recipient a Health Equity Scholarship granting access to the UAID Advanced or Leaders Tier Membership in recognition of their demonstrated commitment to advancing health equity and community impact.

3. Scholarship Term & Benefits

  • This scholarship covers the 2025–2026 academic year, concluding on June 1, 2026.

  • Full waiver of membership fees for the scholarship term.

  • Access to UAID’s advanced programming, mentorship, workshops, and leadership opportunities.

  • Recognition as a UAID Scholarship Recipient on UAID’s website and promotional materials.

4. Recipient Obligations

The Recipient agrees to the following:

4.1 Active Participation

  • Attend the UAID National Conference.

  • Leaders Tier members must also participate in a UAID community project during the term and attend the Winter Symposium.

  • Submit an article to at least one edition of the UAID Journal.

4.2 Public Profile

  • Provide a professional biography and headshot for publication on the UAID website within 14 days of scholarship acceptance.

4.3 Representation

  • Represent UAID in a professional, nonpartisan, and inclusive manner.

  • Participate in external events or collaborations if invited by UAID, when the opportunity reasonably aligns with the Recipient’s schedule.

4.4 Experience Report Requirement

  • Submit a one-page final report by June 1, 2026, meeting the following format:

    • Single-spaced, one-inch margins, 11- or 12-point font.

    • Must include:
      • Summary of participation (events, highlights, key learnings)
      • Description of project or contribution
      • Reflection on lessons learned and leadership growth
      • Future plans for applying the UAID experience

  • Submit the following materials describring your experience as a UAID Scholarship Recipient by June 1, 2026:

    • Quote (150-250 words)

    • 2-3 “In the field” pictures

    • 2-3 minute video testimonial

4.5 Feedback & Survey

  • Complete all surveys provided by UAID.

4.6 Media & Publicity

  • Grant UAID permission to use any materials provided — including the one-page report, quote, photos, videos, and any part of the scholarship application — for marketing, fundraising, or educational purposes in any medium, without additional approval or compensation.

5. Code of Conduct

Recipient shall uphold UAID’s mission and professional standards. Discrimination, harassment, or unethical conduct may result in immediate termination of the scholarship and membership privileges.

6. Termination

UAID may revoke this scholarship if the Recipient fails to meet participation, reporting, or conduct requirements, or acts in a manner inconsistent with UAID’s mission or reputation.  If this occurs, Recipient will be responsible for retroactively paying the full membership fee.

7. Intellectual Property

All projects or materials created by the Recipient remain their property; however, UAID retains a non-exclusive, royalty-free license to reproduce, display, or distribute such works (including excerpts) for educational or promotional purposes.

8. Liability

Recipient participates voluntarily and assumes all associated risks. UAID is not liable for injury, loss, or damages arising from participation.

9. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of North Dakota. Any disputes shall be resolved in the courts of North Dakota.

10. Acknowledgment

By signing or submitting the accompanying form below, the Recipient acknowledges understanding and acceptance of all terms and obligations outlined in this Agreement.

11. Amendments and Updates

UAID reserves the right to update or amend the terms and conditions of this Agreement at any time. The Recipient is responsible for reviewing and complying with any updated terms communicated by UAID during the scholarship term.

12. Acceptance by Submission

By submitting the UAID Health Equity Scholarship Acceptance Form, the Recipient confirms that they have reviewed, understood, and agreed to all terms and conditions contained in this Agreement. Submission of the form constitutes full acceptance of this Agreement and its obligations, regardless of whether a physical signature is provided..
